What causes this?

This happens to me once in a Blue Moon, but I wonder what factor(s) contribute to it. I think it may be:

* Bad construction at the factory
* over/under humidification or temperature
* Smoking too fast

What do you guys think?

Never seen that. Would think that it is construction related more than due to humidity, wind or speed of smoking. At least from what I have seen before.

I have had it happen before, it is caused from tunneling. It tunnels deep and then burns the wrapper I believe.

construction and smoking speed combined, bro. a larger than average airway in the filler, if smoked too quickly, ignited the wrapper from inside before the rest of the cigar could catch up. in my experience, at least.

Someone played an excellent April Fools joke on you.

I've never had this happen to me... but it looks like a result of tunneling (possibly windy where you were smoking?) and bad construction.
