If you are having trouble doing it, here is a technique: As stated by others, blow about 75% (or more, when just starting - so it is perceived as a relaxation friendly gesture and not a painful one) of the cigar smoke directly out the mouth. With what remains, using the tongue and lips in exactly the same manner that you would if taste-testing a ladle of Grandma's soup, at her request, as it sits on the stove. Kind of a internal, roof of the mouth licking/lapping motion with the tongue. This movement forces the remaining smoke into the sinus cavity, without any of it going into the esophagus.
I also feel that immediately after completing this procedure that the foot-generated smoke aromas may be amplified, as well; by rapidly swirling the cigar about 6 inches in front of my face (I think of this as like a carnival "Tilt-a-Whirl" motion) as I'm softly breathing in through the nose. But be careful to keep only the very tiniest amount of smoke from entering the sinus cavity directly from the foot, since that hot smoke can actually numb one's olfactory capacity if too much is ingested directly. Just keep in mind, you are searching for aroma, not smoke.
